インフルエンザ予防接種の予診票は当院受付にてお配りしている他、本サイトからもダウンロードできます。予め記入後お持ち下さい。

  ただし、「65歳以上の藤沢市民」は、当院にて専用の予診票に記入して頂きますので、ダウンロードは不要です。
