民主党の医療政策 第３回

２．地域医療を守る医療機関を維持
民主党は、次のように公約しています。
①地域医療を守る医療機関の入院については診療報酬を増額する。
その際、患者の自己負担が増えないようにする。
②４疾病５事業を中核的に扱う公的な病院（国立・公立病院、日赤病院、厚生年金病院等）を政策的に削減しない（４疾病とは癌・脳卒中・急性心筋梗塞・糖尿病、５事業とは救急医療・災害時医療・僻地医療・周産期医療・小児医療）。
民主党が地域医療崩壊を修復するための措置を講じようとしているのは分かります。
しかし、この公約は公的病院の入院に偏重していると言わざるを得ません。
民主党の「マニフェスト」「INDEX 2009」のどこを読んでも、日本の地域医療の大半を支えている民間中小病院や診療所の役割に対する言及は皆無です。民主党は、救急医療の主役は公的病院であるという誤った認識をしているのではないかと懸念します。
実際には、全国的に見て、救急搬送患者の約６割を民間医療機関が受け入れており、しかも、この割合は大都市圏でより一層高い傾向にあります。
地域医療とは、公的病院の入院部門だけで成り立っているのではありません。
慢性期病院の入院部門や、民間中小病院、有床診療所、無床診療所、さらにはその受け皿となる療養病床等の全ての医療機関が連携してこそ、地域医療提供体制 が構築され、機能を発揮する事が可能となるのです。公的病院だけではなく、全ての医療機関が崩壊の危機に瀕している事への配慮が、残念ながら民主党の公約 には欠けています。
かつての自公政権と同様に、民主党も「医療崩壊」の本質を理解していないのではないかと疑わざるを得ません。
「崖っぷちの日本医療を必ず救う！」と宣言したからには、もっと広い視野と深い知識を持って政策を立案してもらいたいものです。
